Key takeaways

Mounjaro starts at 2.5 mg once a week for 4 weeks, then 5 mg. If more control is needed it rises by 2.5 mg at intervals of 4 weeks or more, to a maximum of 15 mg weekly. It is approved for type 2 diabetes; Zepbound is the tirzepatide brand for weight loss.

How tirzepatide is different

Mounjaro acts on two gut-hormone receptors, GIP and GLP-1, rather than GLP-1 alone. In studies it lowered A1C and weight more on average than semaglutide, although Ozempic has labeled heart and kidney outcome benefits that Mounjaro's US label does not.

The dose steps

  • Weeks 1–4: 2.5 mg (starter, not a treatment dose).
  • Week 5 on: 5 mg.
  • Then +2.5 mg every 4 weeks or more if needed: 7.5, 10, 12.5 and up to 15 mg.

Many people stay on a middle dose for good. Higher is not automatically better if your sugar is on target.

Birth control: an important detail

Tirzepatide can make birth-control pills less effective, mainly after starting and after each dose increase. The label advises switching to a non-oral method or adding a barrier method for 4 weeks after starting and after each step. Ask your prescriber what fits you.

Side effects

  • Eat smaller meals and stop as soon as you feel full — the medicine makes fullness arrive earlier.
  • Go easy on fatty, fried and very sweet foods in the days after a dose increase.
  • Sip water through the day; vomiting or diarrhea can dehydrate you and strain the kidneys.
  • Tell your prescriber if nausea lasts — staying on a lower dose longer is a common fix.

With insulin or a sulfonylurea, low blood sugar is more likely — your other doses may need adjusting.

Storage and missed doses

Keep pens refrigerated; a single pen can stay unrefrigerated up to 86°F (30°C) for up to 21 days. If you miss a dose, take it within 4 days (96 hours); after that, skip it and keep your usual day.

⚠ Boxed warning: risk of thyroid C-cell tumors. Do not use if you or a family member has had medullary thyroid carcinoma (MTC) or if you have Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
