What makes it different

Orforglipron is a small molecule, not a peptide. That is why it survives digestion without the strict empty-stomach routine semaglutide tablets need. The FDA approved it for weight management on April 1, 2026.

The dose ladder

  • Days 1–30: 0.8 mg.
  • Days 31–60: 2.5 mg.
  • Days 61–90: 5.5 mg — the lowest maintenance dose.
  • If needed: 9 mg, 14.5 mg, then 17.2 mg, each after at least 30 days.

If you take a strong CYP3A4 inhibitor (some antifungals and antibiotics, for example), the maximum is 9 mg. Give your pharmacist a full medicine list.

Making it a habit

Because timing is flexible, tie it to something you already do daily — brushing your teeth or your morning coffee. Keep tablets at room temperature in the original container.

Side effects

Gastrointestinal effects are the most common, as with other GLP-1 medicines. The thyroid C-cell boxed warning applies.

  • Eat smaller meals and stop as soon as you feel full — the medicine makes fullness arrive earlier.
  • Go easy on fatty, fried and very sweet foods in the days after a dose increase.
  • Sip water through the day; vomiting or diarrhea can dehydrate you and strain the kidneys.
  • Tell your prescriber if nausea lasts — staying on a lower dose longer is a common fix.

Cost

LillyDirect self-pay launched at $149 a month for 0.8 mg, $199 for 2.5 mg and $299 for higher doses. With commercial coverage, a savings card may bring it to $25 a month.

⚠ Boxed warning: risk of thyroid C-cell tumors. Do not use if you or a family member has had medullary thyroid carcinoma (MTC) or if you have Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
